GP Liaison

GP Liaison

The GP liaison:
  • provides a point of contact for GP enquiries and concerns of a non-clinical nature. As an example, you may have useful feedback around communication processes. Clinical queries and concerns should be addressed to the treating hospital team
  • promotes clear and effective communication between hospitals and primary care providers, particularly in the areas of GP referral, inpatient discharge, outpatient and Emergency Department visits
  • represents General Practice as appropriate during the development of hospital and health service strategic planning, policies and processes affecting patient care and access
  • develops a perspective of Hospital/GP related issues at a state and national level
  • works with the WA Primary Health Alliance and hospitals to provide GP education events and updates.

Hospital GP Liaison

The Hospital Liaison GP works on system issues to improve patient care as patients move between hospital and community care.

Queries from GPs regarding a patient or clinical service should in the first instance be directed to the relevant specialty Registrar or personnel listed within the Hospital GP Liaison section above.

Medical Record Information can be obtained through the NMHS Freedom of Information departments.

Queries from patients or relatives

Queries from patients or relatives

The Liaison GP does not take queries direct from patients or relatives. These should be directed to the relevant Department or the Consumer Liaison Service.

Change in GP details

Change in GP details

Our hospital network maintains a comprehensive database of referring GP details. If correspondence is sent to the wrong address or your practice details change, please advise by emailing, information to be amended to - Central Referral Service Central.ReferralService@health.wa.gov.au.

Central Referral Service can also be contacted on Tel: 1300 551 142 or Fax:1300 365 056 if required.

GP Visiting Rights

GP Visiting Rights

General practitioners are welcome to visit their patients while they are in our hospitals and to review their medical notes. Please contact the doctors of the treating team if you would like to review the medical records. Patient consent should also be obtained.
