When you access this Site, a record of your visit will be kept. The logged information will only be used for statistical purposes.

Information provided via online forms may be stored in databases. Your information will only be used for the purpose for which it was provided.

Your e-mail address will be recorded if you send a message. Your e-mail address will only be used for the purpose for which you have provided it and it will not be added to a mailing list or used for any other purpose without your consent.

Patient collection notice

NMHS is part of the WA public health system. We provide hospital and other community and public health services in metropolitan Perth.

NMHS collects your health and personal information to:

  • provide you with safe and high-quality health care services and other support
  • improve our services
  • complete a survey about your visit
  • facilitate health education, research, teaching and safety and quality improvements
  • carry out our functions as a health service provider under law, including the Health Services Act 2016.

Examples of personal information we collect include your contact details and medical and treatment history, previous test results and current medications.

 

Who outside NMHS may receive information about you?

We may share your information with external service providers so that we can provide you with the best care and organise services for you. This may include:

  • your GP or specialist
  • other external health care providers, such as clinics or another hospital
  • patient transport or community services
  • third parties who help carry out our healthcare service functions.

In addition, we may share information with:

  • organisations funding the provision of our services to you and government departments such as Medicare
  • the Department of Health in Western Australia as part of regular disclosure and statistical reporting, or for specific purposes by agreement between the State of Western Australia and the Commonwealth governments
  • your My Health Record, if you have one, including discharge summaries, pathology and imaging reports. For more information, visit My Health Record.

We will also disclose certain information if required by law, for example:

  • by a Court or Tribunal, such as in response to a subpoena or summons
  • about patients who have specific conditions or diseases to the Department of Health or other government bodies or registries.

 

What are the consequences if you do not provide your information?

If you do not provide us with your personal information or it is inaccurate or incomplete, it may affect the quality and safety of the care you receive, or our ability to provide services for your ongoing care.
